Individual, seminar and mp3 /cd purchase terms and conditions

Note: MP3's and CD's are not intended to be used while driving or operating machinery. Hypnosis can produce a deeply relaxed state of mind and body so be sure to listen while relaxing comfortably at home.

1. I understand that the intended benefits of services are not a state of sleep but a natural state of mind that produces relaxation of mind and body. I understand that hypnosis may transcend the analytical level of the mind and facilitate acceptance of suggestions, directions and instructions desired by me. Hypnosis is simply a heightened sense of mental awareness and relaxation.
2. I am aware that hypnosis can also elicit information and insights from the inner mind. The purpose of hypnosis/hypnotherapy is to achieve freedom from restrictive thought and belief systems to assist in solving personal problems, developing motivation and achieving goals.
3. I acknowledge that hypnosis is not an exact science and that no guarantees are made to me as to the results that may be obtained. I understand that my full cooperation is required.
4. I understand that Terri Cooper is a social worker and is not a physician, psychologist, clinical social worker or medical professional. I understand that the process is incremental and experiential; changes which occur are facilitated by Hypnosis for Life Inc and ultimately change is my responsibility.
5. I am aware that the services given to me hereunder or the product purchased by me are not offered as a substitute for or replacement to any conventional medical care for physical or emotional ailments and that the service and/or products are to be used in conjunction with the knowledge of my attending medical professional(s). Furthermore, I understand that hypnosis is non-diagnostic and does not include the practice of medicine.
6. If I am seeking services for a diagnosed mental or physical illness or pain, I understand that I must have the consent of my physician, psychologist, and any other health practitioner who has diagnosed me, before using this service.
7. I understand that I must be 18 to purchase this service/product. If I am a minor my guardian must consent in writing and be present.
8. Refund policy MP3s cannot be returned and are therefore non refundable. All other products/services may be refunded at the discretion of Hypnosis for Life Inc. on an individual basis. Products purchased are intended for the purchasers use, no mass production or public broadcasting permitted.
9. I have read and understand this document and I hereby release Hypnosis for Life Inc. St. Albert Seminars Inc. Terri Cooper, and all entitites assistants, employees and agents) from any and all liability for injury, loss or damage incurred as a result of any of my use of this service or product purchased or obtained from Hypnosis for Life In c. ,regardless of whether such loss or damage is incurred as a result of negligence or fault of the above named. By my purchase and attendance, I confirm my consent to the above.
