This standard informed consent ensures that you have a clear understanding that:workshops are intended to benefit all those present. As such, a positive presence is necessary. An atmosphere of respect is necessary to facilitate learning. That Terri Cooper is a registered social worker and clinical hypnotherapist, not a physician, psychologist or medical professional. Workshops are not intended to address diagnosed mental or physical illness. That any materials provided are intended for the attendees use, no mass production or public broadcasting permitted. Refund policy-once you have reserved your seat, it is yours, it is not transferable. Refunds may be provided, with an administration fee, at the discretion of Hypnosis for Life Inc and require a medical note. I understand that workshops may not be recorded in any format other then written. Terri Cooper may record all or parts of the seminar. I have read and understand this document and I hereby release Hypnosis for Life Inc. Terri Cooper, and its assistants, employees and agents) from any and all liability for injury, loss or damage incurred as a result of any of my use of this service or product purchased or obtained from Hypnosis for Life Inc.
