Informed Consent

I understand that Hypnosis for Life Inc. is a private social work practice utilizing the following modalities:

• Hypnosis/hypnotherapy
• Solution focused coaching
• Neuro linguistic programming

Benefits
I understand that the intended benefits of hypnosis are not a state of sleep but a natural state of mind that produces relaxation of mind and body. I understand that hypnosis may transcend the analytical level of the mind and facilitate acceptance of suggestions, directions and instructions desired by me. I understand that services can be powerful in terms of altering habits and attitudes, if I so choose to fully engage and commit to the process.

Conduct
I understand that seminars/workshops/group work is intended to benefit all those present. As such, my positive presence is necessary. An atmosphere of respect is necessary to facilitate learning.

No substitution
I understand that Terri Cooper is a registered social worker and clinical hypnotherapist, not a physician, psychologist, clinical social worker or medical professional. If I am seeking hypnosis for a diagnosed mental or physical illness or pain, I understand that I must have the consent of my physician, psychologist, and any other health practitioner who has diagnosed me.

Refunds/Insufficient attendance/Production
Refunds are at the discretion of Hypnosis for Life Inc. on an individual basis. In the event that there are less then five participants registered your registration may be rolled into the next intensive (next month) or you may choose to request a two hour individual session in exchange. Products purchased or provided are intended for the purchaser’s use, no mass production or public broadcasting permitted. I understand that sessions/workshops/seminars may not be recorded in any format other then written.

I have read and understand this document and I hereby release Hypnosis for Life Inc. , Terri Cooper, and its assistants, employees and agents) from any and all liability for injury, loss or damage incurred as a result of any of my use of this service or product purchased or obtained from Hypnosis for Life Inc. regardless of whether such loss or damage is incurred as a result of negligence or fault of Hypnosis for Life Inc., Terri Cooper, or its assistants, employees and agents. By my purchase, I confirm my consent to the above.
